Information for New Students

As you prepare to join the campus community, the wellness team is eager to assist you to maintain good health and wellbeing as you complete your college education.  Submission of health documents and preparation for care while away from home will assist our wellness team to provide high quality care and support for your holistic needs.

The following information and action is required prior to the start of Welcome Week:

  1. A complete Immunization Record and TB Screening Form.
    • Highly recommended vaccinations for matriculation are part of the University’s commitment to health and safety for all. These are based on the Center for Disease Control (CDC) recommendations for adults living in a community setting.
    • Documentation of immunization status of all vaccinations listed on the form are required for matriculation. 
    • Medical exemptions are accepted based on requirements listed HERE.

#1 Tuberculosis Screening Form

#2 Measles, Mumps & Rubella (MMR)

2 dose immunization or positive titer

Other recommended vaccinations (not required for matriculation)

  1. Proof of Health Insurance: Picture of the front and back of your current insurance card.
    • (Sign in to your OneLogin. Click on the 'Point N Click Patient Portal' icon, and navigate to the 'Insurance Card' section on the left side menu to upload the front and back of your insurance Card).
  2. Find local healthcare facilities, such as urgent care and emergency services that accept your health insurance plan. 
  3. Consider transportation options for planned medical appointments and pharmacy needs.

This is done by visiting pl.nu/well, signing in to your OneLogin, clicking on the 'Point N Click Patient Portal' icon, and navigating to the 'Forms and Requirements' section on the left side menu. Please also include a copy of the front and back of your medical insurance card in the 'Insurance Card' section. The deadline for Fall semester is July 1; deadline for Spring semester is January 1.

How do I submit my immunization records?

I am having trouble loading documents into my wellness portal, what do I do?

Can I schedule an appointment at the Wellness Center to receive my immunizations?

We carry some immunizations and also partner with local pharmacies to provide immunization clinics frequently on campus.

What if I do not have a medical exemption but choose not to be vaccinated for other reasons?

How do I know I am compliant with immunization requirements?

As a consumer of student health services, you have responsibilities as well as rights. They include the following:

  • Prescriptive medications, injections, and laboratory tests will have a financial charge. Charges will be added to your school account. The cost is kept as low as possible. It is your responsibility to discuss any financial burden this might pose.
  • Being a good health consumer does not mean being a silent partner. If you feel your rights have not been respected, ask to see the Director of the Wellness Center or the Dean of counseling & student care.
  • Your health is a shared responsibility.
  • Be honest and direct in a courteous fashion about everything that relates to your needs for health services.
  • Be sure you understand your health situation. If you do not understand the treatment plan or test, ask the Provider about it.
  • Follow the prescribed plan. It is your responsibility to advise us if you cannot follow it.
